AVMA/GHLIT offers you the opportunity to start out with a “Pick and Choose” package of basic benefits, at affordable rates, REGARDLESS OF ANY PRE-EXISTING MEDICAL CONDTIONS. If you have eligible family members, certain guarantees are available to them as well.
Below are highlights of the benefits offered, this is a once in a lifetime offer with NO questions or medical underwriting needed -
- Health Insurance (Guaranteed Issue )…regardless of your medical history you will be covered by the trust at the lowest possible premium rates (if you do not sign up for coverage before graduation you will be subject to underwriting and will likely receive higher cost premiums)
- Medical Plans of your choice include coverage for your eligible dependents Health Plans include Traditional PPO, Value PPO, Major Medical, HSAs
- Hospital Indemnity Plan (Guaranteed Issue ) – $100 per day ($400 Max)
- Benefits paid directly to you in the event of a hospitalization, including maternity, for you and your eligible dependents
- Long Term Disability Income (Guaranteed Issue ) ( Paycheck Insurance)…$2,000 per month
- That replaces your monthly income in the case of a disability.
- This benefit includes the Cost of Living Adjustment Option, the “Own Occupation” Option, and Future Purchase Option with or without the Basic Protection Package which includes an Accidental Death and Dismemberment benefit, Decreasing Term Life Insurance Benefit and a Rabies Prophylaxis Benefit which pays up to $600/per occurrence.
- Term Life Insurance(Guaranteed Issue ) – $100,000
- $50,000 for your spouse and $5,000 or $10,000 for each of your children.
- Accidental Death and Dismemberment (Guaranteed Issue ) – $100,000
- Coverage for yourself and $50,000 for your spouse.
- Short Term Disability Income Insurance (Guaranteed Issue ) – $500 per month
- Professional Overhead Expense Insurance (Guaranteed Issue ) – $300 per month
- Dental Plans also avalible from Ameritas Dental – Dental Plans provide coverage to pay the cost of regular teeth cleanings, x-rays, even complex procedures and more..
